Incomplete applications will not be considered. All applicants must provide documentation of 501(C)(3) tax exempt status, must have been in operation no less than two years, and must provide financial statements as required by the Internal Revenue Service. If you are part of a larger organization, the financial summary must reflect your specific business unit.

The Grant Application Form may be filled in on your computer, or you may print a blank form and complete it on a typewriter or by hand. If you fill in the form on your computer, you cannot save the completed copy on your computer. You must print it and save copies of the completed form.
